80 year old female with infected TKA

5 years old left TKA, st post ORIF of left patella-hardware taken at index procedure- asymptomatic until 6 months ago when she was diagnosed acute onset septic arthritis. Staph epider. was cultured from sinovia. Treated on the 4. day of pain onset by arthro lavage & continuus lavage-antibiotics i.v. for 14 days then perorally since-by antibiogram. Was fine for a couple of months but returned with pain while walking and on climbing stairs, CRP 30, knee warm. Scintigraphy with Indium labeled leuko. positive, uploaded The prosthesis stable initially and know. Scrutinizing the available and uploaded imaging and having the patella being operated on before, my working diagnosis is osteomyelitis of the patella. Im thinking of patellectomy and implant retention although is 6 month after the acute onset, but with antibiotic therapy (ciprofloxacin- rimfapicin) all the way. Much obliged for any suggestion.
