34y old male, carpenter. 2008 MVA, proximal humerus #(dislocation), plate fixation. Presented to me 2013. he had good ROM- 140 degree flexion, with compensation 170. L spine IR, ER was limited -10 degrees. no pain, could work, could do push ups. Xray showed a healed fixation of the fracture, but glenohumeral posterior luxation (missed), with huge Reverse Hill Sachs lesion. we decided to wait because of the lack of symptoms, CT checks done every year since, there is no change since. The patient 8 years after the accident starts to feel some pain during physical activity, ROM is the same. lack of ER is a disturbing factor lately.
