Failed ACL reconstruction

By Alper Muradov

Hi,I saw a 22 yrs old male who had 3 previous surgeries of the same knee.2010 ACL recontruction with quadrupled STG autograft fixed with RigidFix.Next year the patent underwent arthroscopy for Cyclops syndrome.Last operation was new ACL reconstrucion with STG autograft from contralateral knee, fixed with resorbable screw on tibia and endobutton for femoral tunnel.
On the examination this patient have Lachmann 2+,Pivot shift 2+ and Appley + test.
MRI shows medial meniscus rupture,Graft faillure, chondropathy of the medial knee compartment 2 degree and widening of previous tibial tunnels as written below:
Tibial tunells: first operation 10 mm wide and 30mm lenght.Second reconstruction tunnel:11mm witdth and 41,6mm lenght.
Femural tunel 8,5 mm width and 31mm lenght.

I am thinking what will be the best solution for this case according to that the patient is very young and active and want to participate in sports activities like football, ski on non professional level. Are we have to do 2 stage operation or we can manage this case with single reconstruction?
What about using BTB allofraft.
