24 y.o. Male with Hip Heterotropic Ossification and Subluxation following Anatomical Reduction

By Abdullah Hammad

I have a 24 years old male who had a transverse posterior wall acetabular fracture and hip dislocation.Surgery had to be postponed for one month because of infected suprapubic cath.Initial reduction was anatomical but the patient developed AVN very rapidly. Moreover, Heterotropic ossification grade 3 had evolved over 9 month period leaving the hip in almost 70 degree flexion deformity. The head is showing resorption and is subluxed What to do? CRP is negative and I can say he is not infected.Should I do THR and excision of HO in the same setting or should i do arthrodhesis
