Proximal Humerus Fracture in Active 70 y/o Male

By Kevin Forsythe

70 y/o male avid cyclist, had a fall from bicycle sustaining proximal humerus fracture shown (isolated injury). With all of the comminution, I'm tempted to recommend arthroplasty, but I'm concerned that a reverse may not fare well with continued cycling. Hemiarthroplasty perhaps more durable, assuming I can get enough purchase on tuberosity fragments and get them to heal. I suppose if the hemi fails, the system uses the same stem for the reverse, so a conversion to reverse later would be more reasonable. Appreciate any thoughts, I'll be seeing him this week.
