Overview

Expanded indications for cardiac implantable electronic devices (CIEDs) have led to more recognition of related complications including lead malfunction and infection which often require transvenous lead extraction. Properly managing these complications can be critically important and even lifesaving for these patients. New device technologies including subcutaneous devices and leadless pacemakers present new challenges for device extraction. New endovascular leads or epicardial leads represent their own obstacles. This symposium will focus on the rapidly changing landscape of CIED and lead management including review of state of the art practices to predict and minimize infection, overview of best practices and techniques for performing lead and device extraction. 

Objectives

After participating in this activity, health care providers will be able to do the following:

  • List the incidence, outcomes, and risk assessment of infectious pathologies encountered with cardiac implantable electronic devices (CIEDs).
  • Describe best practices to prevent CIED infections based on recent landmark trials.
  • Discuss the indications, risk assessment, and pathology associated with CIED extraction in specific clinical scenarios.
  • Identify current and evolving techniques for performing lead extraction.
  • Review the published evidence on indications and techniques for leadless pacemaker retrieval and describe their use in clinical practice.
  • Critically appraise the pros and cons of laser-powered versus mechanical lead extraction including a review of devices, techniques, and timing.
