One of the main issues still hindering such evolution is the vascular complication which is documented in various trials to be directly related to mortality. With continuous industrial effort to minimize the delivery sheath sizes we should also try to improve our techniques by minimizing the possibility for vascular complication. Our case is a transfemoral TAVI procedure using a self-expandable valve. We demonstrate tips for utilizing a single radial access for guiding the femoral puncture, inserting a protection wire inside the femoral artery and doing the aortography during valve deployment utilizing only the same access which has a lower vascular complication in comparison to contralateral femoral access.
