Failed DHS for HRA. How to tackle the tissue contractures and bring the hip down?
A patient was operated by a colleague of mine approximately 8 weeks back for comminuted fracture of intertrochanteric region. Patient had another fall showing implant failure for which he took the patient for a bipolar prosthesis fixation following DHS extraction. He could not bring the hip down post-extraction on table and was in a fix. He sent me a pre-operative and a post-operative radiograph which are attached below.
I asked him about the soft tissue releases to be performed to bring the hip down which I believe he did not follow. Kindly suggest the ways to bring this hip down.
