22 year old female with patellofemoral pain

I am seeing a female 22-year-old former soccer athlete now in the work world who injured her knee in August of 2012 telling me that it was a medial collateral ligament sprain. She never recovered from the pain that developed and had peri-patellar and medial joint pain for which she had an arthroscope in Colorado in January 2014 with the only apparent finding a plica. She is athletic, she exercises, she has worn braces, anti-inflammatories, and continues to be unhappy with her knee. Recent MRI was entirely normal.

Exam shows normal medial retinaculum stability and not an excessively tight lateral retinaculum with a mild Q angle. Most pain is anteromedial and medial side of patella. She has good musculature in legs.

I have suggested to her that there are not great surgical alternatives for changing her knee, and it would probably be best she accept what she has. However she continues to come back and be unhappy.
