read more ↘ can be only be attributed to the fury of Mother Nature (The Universe expressing itself in the most gross and most subtle forms). Every act of this Universe is a mystery and a miracle. The power of ‘thought’ is as powerful as the disease itself and we have seen the extent of fear and other emotions this has raised; hence, it is important to choose what we think, hear, speak and do in such disaster times.
-It is spectacular to see how the entire world is coming together as One in the name of Coronavirus. However, we need to pause for a moment and understand the true face of this epidemic without emoting (using the intellect rather than the mind). Having an open approach to a novel problem is essential because we as ICU doctors saw that our presumption of Covid-19 respiratory failure as ARDS that usually affects lung compliance and responds to PEEP is being questioned now and is influencing our decisions and outcomes. Our open-minded search for the mechanism of this disease is unfolding as we speak. Similarly, any event on Mother Earth has 2 sides to it (or a spectrum); the good and bad. We are starting to look at the coronavirus as a tool rather than a disease that Nature has given us and are trying to understand how to convert this ‘bad’ into ‘good’; how to tap the whole potential of our human body and life by balancing our interpretation and reaction to this virus and outcomes of the disease.
For this interpretation, we are grateful for the wisdom from our ancestors across 1000s of years on Mother Earth. The universe and earth are gross manifestations of our inner microcosmos established in each human being; we are a continuum. The core of a human being is the ‘unified field’ that ultimately gives birth to all 5 natural forms of energy in our body – air, water, fire, space, earth. Events in the outer world represent the ups and downs in the gut of a human being or the nucleus of each cell in our body. Our gut or core is very sensitive to all forms of interaction with the environment inside and outside of it. We know modern medicine defines disease as a complex interaction of the disease-causing agent, the host and the environment. Since about 95% of the people who test positive for the virus recover from the symptoms, it does indicate that most of us have something inherent already in our human inner environment that creates a symbiotic relationship with this virus. For instance, researchers are trying to use plasma from survivors to see if that can prevent deaths in other patients who are potentially missing that secret ingredient. Now, we know the home of our immunity is indeed in the gut and the microbiome that is flourishing in it. Hence, the lesson of this virus epidemic from Mother Earth may be to urge the entire humanity to understand how we can boost our innate immunity, gut microbiome and natural defense for that 5% of the population and for future generations. This is the goal of the epidemic and is the duty of each one of us in the public, while physicians continue to try to save the 5% of population now. For this we as physician researchers are looking into tapping the plasma from the blood of healthy and milder covid-19 patients and also the nature of the gut immunity to see if we can slow down the process of the viral attack.
For the general public (survivors and mild covid patients), again, let’s go back to the wisdom from our scriptures including The Gita, The Ayurveda or Vedas from India, or even from other scriptures from Egyptian, Chinese, European culture. Health comes from striking a balance in the natural forces that drive our desires (‘vasanas’) and duties(‘dharma’). This epidemic is an outstanding case example given by Mother Nature for all humans to check this concept of health for ourselves in real time. Each of us is stuck inside our homes at various levels depending on our duties towards self (age, sick with virus, immunocompromised, self-protective), duties towards society (people who want to avoid spreading the virus, healthcare or essential workers), and desires (to help like Uber drivers, restaurants). Every moment during this epidemic, we go thru this constant analysis of what is my role today since I am here. As a physician in the ICU at North Central Bronx hospital at Albert Einstein College, Bronx, I executed my duties taking care of the sickest patients during the peak covid-19 NYC season from March 23 to 29. I went through a roller coaster of emotions due to the severity, rapidity and uncertainty of the disease and therapy. Thanks to the excellent teamwork from the ICU staff all the way to the state authorities and the public, we are going strong every day trying to learn the disease and making our critically ill loved ones survive. During the busy week, I kept frequent checks on the balance between the duties and desires (as a physician, as a team member, as a human). Fulfilling the duties towards my own health alongside the duties towards patients and the nation made my experience much more enlightening. On my day off after the busy week, I had constant desires to keep track of some of the patients; I was physically at home, but not mentally; this caused some discomfort because it affected my duties at home as a husband and a human. That’s when The Gita Chapter 2 and Mahabharata war came to my rescue. Being present and executing the duties at the present moment bestows peace and health in the individual and the family. This form of journaling helps me a lot in my personal growth (‘ideal human’ or stitha pragnya) and I am having a mind-stirring experience trying to balance the desires to help areas of need in my hospital, desire vs duty to study these patients, publish and spread knowledge to the society about our patient experiences. This exercise is a consistent feature in our daily lives and is exemplified already in the scriptures, world wards and previous epidemics. A fitting synonym for this ideal human experience are: Nishkama-karma (Detached attachment). I am pausing every 20 minutes so that I can intellectualize that I am respecting my duties towards my health and my home (including planning my cooking, errands, resting and eating before I get hungry), my duties towards patients and local medical fraternity (sharing insight and gathering data on coronavirus, ensuring nourishment of staff) and my desires (reading newspapers, trying to publish, recording music). This balance between work and life is key to health and immunity. Keeping a ceiling and balance on our individual desires is paramount to protecting our health. When a spouse/friend requests you to join him/her for cooking, exercise or other errands (duties), we may not realize that those are protective of health; our mental instinct usually is following the desire to finish complete our endeavors. Especially, now that most of the public is at home, this is a chance to practice the duty-desire tight-rope walking.
